Medications/Inhalers/Epi-pens/Health Plans
Forms must be completed every year.  Information and required forms can be found here.  Please do not bring large quantities due to lack of available storage space for medicines.  Please put your student's first and last name on all non-prescription medicine.
All medications brought onto school grounds must be in their original packaging, with the pharmacy label (prescription) or package instructions (nonprescription) clearly intact and readable.
