Injury Reporting Procedures

  • If you are injured on the job:

    Not immediately seeking medical attention

    1. Call Human Resources at 323-4056 WITHIN 24 HOURS. Leave a message.
    2. Fill out the Employee Injury Report and notify your supervisor.
    3. Once employees notify Human Resources, the employee has two weeks to file a claim and seek medical attention.

    Immediately seeking medical attention

    2. Fill out the Employee Injury Report and notify your supervisor.
    3. Employee must use the medical providers listed under the Designated Medical Provider program:

Designated Medical Provider (DMP) Program Information

  • Bismarck Public Schools is part of the Designated Medical Provider (DMP) Program under Workforce Safety and Insurance (WSI) law. If you are injured during the course of employment with BPS and need medical care, you need to seek medical attention with one of the following providers:

    1. Sanford Health Occupational Medicine Clinic

    1. WorkLife at MidDakota Clinic
           401 N 9th St, Bismarck, ND 58501 – 530-6400

    If you need medical care for a work related injury, you must go to a designated medical provider. If you go to another provider for care, WSI may not pay for the medical treatment unless it was an emergency.

    If you go to one of the DMP’s, you can be referred to someone else for treatment if needed. Please go to the DMP first, even if you think you will need to go to another provider for treatment.

    If a doctor you prefer is not associated with either one of the district’s DMPs, you may add their names to your original DMP form. You must list your selection prior to a work related injury. Even if you select and list an alternative medical provider, you are still authorized to use the district’s DMPs. Emergency care is exempt from the DMP requirement.
