Please complete this form if you are considering the purchase of more than 10 licenses of a particular software program, subscribing to an online website that will be used by more than 10 students at a time, or looking at textbooks that include software or an online environment. Technology will not support the software or online environment if it has not been approved through this process. This is a long process and can take several weeks before an approval/disapproval of the software is made. All questions with an asterisk are required.
